As of January 1, 2026, Hometown Health will no longer offer an Individual & Family PPO plan. Members currently enrolled in an Individual & Family PPO plan will be offered an alternative EPO (Exclusive Provider Organization) plan.

To continue to provide high-quality affordable plans that better meet the needs of our members today and into the future, the decision was made to sunset the Individual & Family PPO plan and encourage members on that plan to select an EPO plan option,
The current Individual & Family PPO plans will end coverage at midnight on December 31, 2025.
-A letter will be mailed on 9/1/2025 providing details
-Multiple emails between 9/15/2025 and 12/1/2025 will be sent providing details
- A comparable EPO (Exclusive Provider Organization) will be offered to you during the annual open enrollment period beginning on November 1, 2025
Beginning November 1, 2025, an email with a link to your renewal will be sent with the alternative plan and all plan options
You will either need to select the comparable EPO plan or any other available plan and submit your choice before December 31, 2025
You must select a renewal plan by December 31, 2025, for your coverage to continue
If a renewal plan is not submitted coverage will terminate January 1, 2026
- Some providers such as Barton Healthcare and Tahoe Forest will no longer be in network for your new plan. Please refer to the EPO directory https://apps.hometownhealth.com/onlineproviderdirectory/en?network=Nevada%20EPO%20Network to review in network providers.

- – November 1, 2025-December 31, 2025: Choose your plan to start January 1, 2026
- – January 1, 2026-January 15, 2026: Choose your plan to start February 1, 2026
