Primary Care Provider Online Change Form

Changing your Primary Care Provider (PCP) is quick and easy. Follow the simple steps below to change your PCP. You will want to have your Member ID Card handy as you will need to enter your member ID number.

Step 1: Visit the Online Provider Directory to search for a new PCP.  If you already know the name of the new  PCP you would like to select, you can skip this step.

Step 2: Click the “Change Your PCP’ Link below and complete and submit the “Change Your PCP” online form.
